Unfortunately, Dr Jeremy Lim is not able to be your GP. Everyone should have a trusted Specialist General Practitioner who they see regularly for in-person review of acute ailments, health check-ups, and chronic disease management. Dr Lim takes an integrative approach to medical care and aims to work in synergy with your other healthcare providers.

1) Book an appointment via the Halaxy link. 2) Fill out our Intake Form and upload your recent Health Summary on Halaxy. 3) Your medical issues and treatment goals will be reviewed during the consult. 4) Any scripts needed will be sent to a pharmacy of your preference. Follow up treatment plan provided. 5) Please inform Dr Lim if you would like him to provide an update to your regular GP.

No, you do not. However, please submit your most recent Health Summary to me on Halaxy for a holistic understanding of your medical issues. With your consent, Dr Lim may also access your My Health Record profile for additional information.
+ "Am I eligible to access alternative medical treatment?"
Dr Lim will decide during the consultation whether you are an appropriate candidate for certain treatment plans. Refunds are provided if you are not a suitable patient to work with Dr Lim via Telehealth.

These Terms and Conditions govern the access to and use of the services provided by FFS, a smoking cessation program under the ownership and operational control of Whole Being Medical, which is owned and operated by Dr. Jeremy Lim ("WBM"). By accessing or utilizing the services offered by FFS, including booking an Appointment, using our Website, or using our Services via Halaxy, you ("The Patient" / "You") hereby agree to be legally bound by these Terms of Use, along with our Privacy Policy. Should you not agree with any provision of these Terms and Conditions, you are required to immediately cease using our services.
All references within these Terms and Conditions to "FFS," "we," "us," or "our" shall be construed to mean Whole Being Medical and its associated entities, under the direction of its medical ownership.

Fees

Consultation fees will at all times correspond to the fees as listed on the FFS Website. For the avoidance of doubt, Medicare rebates do not apply to FFS consultation fees. The Patient acknowledges that Halaxy may impose a processing fee, currently set at 3%, which is additional to the stated consultation fees.We do not offered any refund for cancellation within 24 hours of your requested appointment period.

Patient Conduct

The Patient agrees to conduct themselves in a respectful and responsible manner at all times when interacting with FFS staff and Dr. Lim. The Patient bears the responsibility for diligently following any treatment plans or recommendations provided by the clinic. FFS reserves the explicit right to decline the provision of further care and to refer a patient to an alternative practitioner should the Patient fail to conduct their treatment in a respectful and responsible manner.

Patient Responsibilities

Should the Patient elect to engage with FFS for treatment, the Patient shall bear the following responsibilities:
(a) To provide accurate and comprehensive information pertaining to present complaints, past medical history, medications currently being taken, allergies, and any other matters relevant to their health.
(b) To conduct themselves with consideration and respect towards all Freedom From Smoke staff members.
(c) To accept full responsibility for any consequences arising from their actions should they fail to comply with the rules and instructions provided by FFS and its practitioners.

Personal information & Communication

By accessing FFS, the User explicitly consents to the collection, use, and disclosure of their personal and sensitive information in accordance with these Terms and Conditions and our comprehensive Privacy Policy. Specifically:
(a) The User grants consent for FFS to utilize their information for the purposes of managing appointments, providing medical treatment, and facilitating communication related to their care.
FFS will, as a general principle, only initiate contact with the Patient in instances where the Patient has contacted FFS first or to provide a direct response to the Patient's inquiries or requests. FFS undertakes not to publish, share, or sell the Patient's personal or contact information to any third party without the Patient's explicit written consent. FFS utilizes a patient management system, namely Halaxy, for the purposes of communication, appointment scheduling, and the provision of treatment guidance
(b) The User acknowledges that while the clinic operates under medical ownership, referrals to other healthcare practitioners and service providers, who are independent entities, may be made as deemed necessary for the User's comprehensive care plan.
The provision of personal information to FFS signifies the Patient's explicit agreement to its collection, storage, use, and disclosure as meticulously delineated within the FFS Privacy Policy, accessible [HERE]. Whole Being Medical, under the direct oversight of its medical ownership, remains steadfastly committed to upholding the privacy of its patients and ensuring the secure management of their personal information.

Website and Service Usage

When utilizing the FFS website and services, the Patient explicitly agrees to the following stipulations:
(a) To employ the website and services solely for lawful and legitimate purposes.
(b) To furnish accurate and current information at all times when utilizing the services.
